Dear Parents:
For the upcoming school year, we will be shifting all student medical information, including the annual
sports participation packet, to an online database. There are many benefits to this change including less
paperwork for parents to complete each year and having more accurate information that is accessible to
parents at any given time. The management system we have adopted is called Magnus Health SMR
(Student Medical Record). As this is a web-based system, you will have continuous access to your child’s
health record as well as the ability to make updates when needed.
There are the typical forms to be filled out and signed, but this can be done through e-signatures this
year. With that said, the preparticipation physical is the one form that will still need to be printed out
and taken to a physician to be completed. Once the physician signs and dates it, it can then be uploaded
into your Magnus account via the website or the Magnus health app by taking a picture of it. This will
cut out any problematic areas like lost paperwork, as well as always allow you to see what is turned in or
missing at any time so there are no surprises.
The sports packet will contain the following:
-Consent to Treat and Parent Permission (e-signature)
-Birth Certificate (This will need to be uploaded even if it has been submitted to the Athletics office in
the past.)
-Athletics Fan Agreement (e-signature)
-Preparticipation Physical Requirement (Print, sign, and resubmit)
-MTBI and Concussion Awareness Form (e-signature)
-Brainbook Completion Acknowledgment (For HS Students Only)
